A Clinical Audit on Pneumococcal Vaccination for Patients with Chronic Obstructive Pulmonary Disease followed up in Primary Care Clinics

Introduction: :
Pneumococcal vaccination has been found to reduce community-acquired pneumonia among chronic obstructive pulmonary disease (COPD) patients and reduce the risks of acute exacerbation of COPD. Evidenced-based guidelines recommend pneumococcal vaccination as part of the management of stable COPD.
Objectives: :
To audit percentage of COPD patients followed up in 8 NTWC GOPCs with pneumococcal vaccine given.
Methodology: :
A retrospective review of immunization records of our COPD patients for the First cycle in June 2018 (Reporting period: Apr 2017 to Mar 2018) and the Second cycle in December 2018 (Reporting period: Nov 2017 to Oct 2018). Inclusion criteria included COPD patients aged 40 or above, with GOPC attendance(s) coded with ICPC-2 of 'R79-Chronic bronchitis' or 'R95-Chronic obstructive pulmonary disease' over the 12-month reporting period. Exclusion criteria included GOPC patients with Medical or Respiratory Specialists Outpatient Clinic (SOPC) attendances which were prescribed with pre-defined drugs such as bronchodilators in SOPC during the reporting period.
Result & Outcome: :
There were 1258 patients in the First cycle and 839 patients in the Second cycle. The percentage of pneumococcal vaccination improved from 44% to 59%. Among patients aged 65 or above, the pneumococcal vaccination coverage increased from 55% to 74%.
