I am a registered nurse who works in the Community Outreach Service Team (COST) in Prince of Wales Hospital. Besides assisting Advanced Practice Nurses to lead the team for providing advanced nursing practice, I also provide orientation to new comers in COST.

As a resource person of Diabetic care and Renal care for nursing staff in the team, I share the updated advance nursing practice with colleague in these two areas. Joining home visit with junior staff will be implemented for complex cases, so as to enhance diabetic and renal management of the team members for better patient care in the community. I also participate in various programs to provide post discharge support to patients with an aim to keep them healthy in the community. These programs include ‘Flu Program’, ‘Frequent Admitters Support Program’, ‘Supported Discharge Program for Patient with Heart Failure’ and ‘Geriatrician Front Door Support’ and etc.

The experience of attending the Overseas Corporate Scholarship Program for Clinical Leaders (2017-18) in UC San Diego Health Sciences International broadened my horizons in leadership of healthcare. Now, I am leading a Diabetic group in my team for providing a better patient care and education for diabetic patients in the community.
