Introduction :
Nurses play an important role in care delivery of surgical patients pre-operatively & post operatively. Pre-operative preparation is vital for patient safety, and also ensures the overall operation substantially.
Based on the “Safety Statistic Report in OT reception Aug to Dec 2017”, patients were not adequately prepared for operation. We undertook a program focusing on the surgery delay components to implement strategies to optimize the pre-operative preparation, and therefore prevent the first start delay for surgery
Objectives :
-To ensure patient safety
-To facilitate staff in pre-operative checking
-To enhance communication
-To minimize delay of operation
Methodology :
Diagnostic: Collect colleague’s opinion & analysis the problem of existing forms.
Solution design:
Revised the existing checklist. Implementation:
Approved by COSs & DOMs, briefing sessions & pilot study in pilot wards
Monitoring:
Perform compliance audit on the revised form

Retrospective documentation audit, Staff satisfaction survey & Safety Statistic Report in OT reception

Result & Outcome :
Pilot study was conducted from 27th-31th December 2018. 19 cases were collected. Compliance rate of OT-PPP form and Pre-operative Checklist was 99.7%. Only 3 samples did not complete “Date & time”. Safety Statistic result of the pilot study showed patients were adequately prepared for operation. The result of staff satisfaction survey and safety statistic to OT in Dec 2018 showed no report of surgery delay from OT. All staff on the pilot study was satisfied with the revised forms. The forms were able to enhance patient safety and minimize delay of operation.
