Nurse Competency Enhancement Program For Managing Herpes Zoster Ophthalmicus Patients In An Isolation Ward

Introduction :
Over 80% of the Facial Herpes Zoster cases admitted in an isolation ward (QEH/C10) in 2017 were Herpes Zoster Ophthalmicus(HZO). For providing quality care to HZO patients, an enhancement program was initiated in June 2018, aiming at enhancing nursing staff knowledge, patient education and standardizing care workflow.
Objectives :
1. Set up a standardized quality service management program for HZO patients. 2. Provide clinical guidance, up-to-date information and knowledge of HZO to nursing staff. 3. Design an educational pamphlet to patients. 4. Set up protocol for eye assessment, nursing care procedures for eye hygiene and techniques for eye medication administration.
Methodology :
Educational talks were given to all nursing staff in Jun 2018 with following: 1. Designed patient educational pamphlet. 2. Standardized a quality management program for daily assessment of visual acuity, fall risk and pain control. 3. Standardized nursing procedures of visual acuity assessment; eye hygiene care and eye medication administration. 4. Set up a nurse self-learning corner. 5. Prepared educational materials of HZO and its complications. 6. Refined assessment tools for visual acuity test. 7. Set up a designated eye medication box for individual patient. For evaluating validity of program: 1. Staff knowledge on HZO was tested pre and post talks were recorded. 2. Satisfaction questionnaires to HZO patients were completed during the pilot period(July to October 2018).
Result & Outcome :
Result: All 14 nurses in C10 ward were empowered by increasing their knowledge of HZO and its complications; as well as standard techniques of visual acuity assessment, eye hygiene and eye medication administration. The mean score of the pre and post-test were increased from 45.1 to 90.2. 100% of the participated nurses agreed that the educational talk was useful, practical and achieved the stated objectives. Besides, during the pilot period, 22 patients were recruited for satisfaction questionnaires. 100% of them agreed that the nursing care provided was appropriated and professional. 90% of them agreed that their knowledge and self-care ability were enhanced. Outcome: The program could definitely promote evidence-based nursing knowledge and practices as well as enhancing quality patient care. Patients benefited from receiving standard quality nursing care and health information. Similar structural program can be extended to other prevalent disease entities for quality patient care.
