Introduction :
Cases of child and adolescent psychiatric service users (defined as age under 18 as below) had risen in recent years. Despite the child and adolescent psychiatric service in KCC cluster is still currently under development, those youth service users are required in-patient service which they emerged crisis or unmanageable care in community setting. With increase of their in-patient service demand, the psychiatric rehabilitation ward will be considered as one of the target wards that provide in-patient youth service users. The youth towards admission in a psychiatric ward may sometimes create fears and ambiguity about the ward environment due to unknown of its purpose of hospital treatment and what they will encounter during the process. The adjustment reactions will be created towards their parents or care givers as well. The provision of a welcome package is expected to facilitate their understanding of the ward environment, professional team, activities and goals.
Objectives :
1) The welcome package is regarded as a tool which not only providing useful information, but also empowering the service users as well as their care givers. 2) The youths can increase their positive experiences in the hospital stay and in return better in their recovery journey. 3) The youth service users and care givers will better know the ward environment and the team professionals.
Methodology :
The youth service users are recruited in August to survey their needs about what they wanted to know or about the treatment. For the therapeutic environment, a relaxing home-like environment with common area is considered. Some articles, sofa chairs and bears that suitable for the child and adolescent ward living environment are prepared. Besides, child dormitory is separated which adult patients are forbidden for the sake of safe. For the setup of therapeutic activities, selected board games, expressive art and music elements are adopted. The condemned trolley in ward is converted as a music cart. On the other hands, the draft of information leaflet “青少年入院簡介” was prepared in November 2018. The youth service users are invited to give comments and suggestions in December 2018. A self-designed satisfaction survey is distributed to collect users and their care givers feedback after one-week in-patient service.
Result & Outcome :
A task force group with specialty nurses is set up in July 2018. The group framed out the package content and ideas about the environmental design with reference to the objectives. The context of welcome package involves (1) an information leaflet, (2) set up of suitable activities for youth engagement, and (3) make up therapeutic environment especially for youth. The child dormitory is decorated in October 2018. One user gave feedback feeling him warm especially his LEGO works beign sticked on the wall-mount in the common room. Besides, the toy cart and music cart are available for use. Musical instruments in the cart, such as cajon and maracas can facilitate the youth to increase their stimuli. Service users were also invited to decorate the music carts by sticking the self designed poster sheets in preparation stage. Furthermore, art materials like brush painting and skin painting can stimulate youth creativity and expression. The information leaflet published in January 2019 and distributed to the users and their care givers before the case is transferred to psychiatric rehabilitation ward. On the other hand, the package resulted in youth service users more compliance in in-patient treatment by clinical observation. The users also reflected that the activities help them in stabilizing their emotion in return better mental well-being.
