Introduction :
The incidence of hospital acquired pressure injury (HAPI); is a nursing quality indicator of HA. The HAPI incidence of IDC X ward was detected from July to September 2017. The condition was worsening with significant increase in November 2017. Although preventive measures had reinforced to ward staff, incidences were still detected in February and April of 2018. In order to ensure a safe and quality patient care, a workgroup was set up to formulate intense preventive, monitoring and enforcement strategies in prevention of HAPI in May 2018. A staff compliance monitoring and measurement tool was developed. Education was provided to nurses and care-related support workers to highlight important existing measures and introduce newly developed measures.
Objectives :
To achieve a sustain reduction in the incidence of HAPI by 70% after 6 months of the program
Methodology :
- Establishment of a workgroup - Formulation of preventive, monitoring and enforcement strategies eg. i) Risk assessment ii) Daily skin inspection iii) Turning skills iv) Application of skin care products v) Application of pressure reducing / relieving devices vi) Special care for medical devices that pose risk of pressure injury vii) Encourage and assist patient to mobilization viii) Education to patients and care givers - Promulgation and education to nursing staff and care-related support workers - Monitoring and enforcement of compliance
Result & Outcome :
There was a sustained reduction of HAPI from the highest rate 4.386% per 1000 patient bed days to 0% for eight consecutive months.
