Introduction :
It is a growing in demand of community nursing care and the community nurses are working independently in large remoted area in NTWC. The effectiveness communication intra and inter the teams is important but still not feasible. Use of ‘Hospital Authority (HA) Chat’ to provide timely response to change of patient’s conditions by community nursing team with clinical team to promote promptly communication and clinical consultation is implemented in NTWC CCD since June 2018.
Objectives :
1.To reinforce the using of ‘HA Chat’ network within Community Care Division and clinical teams in NTWC. 2.To review the effectiveness use of ‘HA Chat’ for tele clinical consultation in differences programs. 3.To evaluate the satisfaction of community nursing and clinical team colleagues in HA Chat clinical care support.
Methodology :
1.Two sessions for introduction on using of HA chat were arranged in CNS staff meetings in May 2018. All staffs’ mobile phone or Mini i-pad were installed with ‘HA Chat’ Apps before June 2018. 2.CNS Centers of three areas (Tuen Mun, Yuen Long and Tin Shui Wai) developed their own Chat Group for nursing and clerical staffs to facilitate the internal team communication. 3.Introduced the Chat group concept with two clinical teams, TMH Geriatric Team and POH Cardiac Team. HA Chat groups of Infirmary Care at Home (ICAH) and POH Community Congestive Heart Failure(CHF) program developed and started clinical communication through the Chat groups. 4.Satisfactory survey was carried out after six months of commencement.
Result & Outcome :
1.Chat group for CNS staffs facilitated the safe communication in-between the teams for clinical information related to daily operation like patient’s home environment, updated medical records and location of patient’s home were stored and communicate via the Chat Group in the three CNS Chat groups. 2.Chat groups of individual programs were created. Both community nurses used HA chat to collaborate with clinical teams (TMH Geriatric Team and POH Cardiac Team) to enhance the patient’s consultation in both programs. The complicated patient’s clinical condition images and video were stored and sent to clinic teams for real time consultation. Timely clinical treatment or immediately adjustment of medication will provide through the chat group. 3.The satisfaction survey showed that 89% (N= 73) of community nurses and clinical team (Community Nurse 58, Clerical staff 5, Doctor 3, Clinical Nurse 5, Allied Health 2) agreed the ‘HA Chat’ was user-friendly and give them well operational and clinical support. Conclusion: Use of HA chat is feasible to enhance care support in Community Care. It helped to provide real time, fast response, on-going patient status to facilitate the review of therapy and care plan. Both in-hospital clinical team and Community nurses agreed this is an effective and convenient way to access updated patient condition
