Introduction :
There are 16 clients out of 90 clients are on physical restraint with various reasons at Tin Ka Ping Infirmary (TKPI). The majority of clients are on nasal gastric tube feeding and lack of comprehension. Some of them are even with tracheostomy tube. But client’s safety and dignity are of our concern. A pilot program on Trial Reduction in using Physical Restraint at Tin Ka Ping Infirmary was implemented in March 2017.
Objectives :
1) To enhance patient’s safety while dignity is respected.
2) To minimum the use of physical restraints in infirmary.
Methodology :
Trial Reduction in using Physical Restraint was on trial run from the period 7/3/2017 to 31/03/2017. The program is divided into 2 phases. Phase 1: choose target patient who are on physical restraints and soon will change nasal gastric tube before the scheduled date. If failed to remove physical restraint, then nasal gastric tube will be re-inserted again, and it will not cause any further harm on them. Phase 2: choose patient who do not have any life sustaining devices. If failed to remove physical restraint or limb holder, alternative devices will be considered to apply such as mittens or protective cushions.
Result & Outcome :
Successful cases are 2 out of 4. One male client was successfully removed all physical restraint. And one female client is putting on protective cushion on her right hand in order to prevent her to rub her eyes. Conclusion
Feedback will be collected from the staffs’ comment and documentation on weekly basis. Positive feedback is received that client is less agitated because of the use of physical restraint. Staff education is enhanced on the proper use of physical restraint and is continuously monitored. The project has enhanced staff’s motivation and active participation to try removing patients’ physical restraint. And communication between relative and nurses’ are enhanced.
