Use of Ipad video demonstration to improve Inhaler technique in GOPC

Introduction :
Chronic Obstructive Pulmonary disease (COPD) or asthma is not uncommonly seen in General Out-patient Clinic (GOPC). It is well known that poor Metered-dose Inhaler (MDI) inhalation technique was an important cause of suboptimal disease control. Study has showed that 90% of patient showed incorrect technique with either standard pressurized MDI or dry-powder inhalers. While inhalation technique is important in achieving optimal drug effect, in our busy GOPC setting, it is not easy to educate patient very clearly or to review their inhalation technique.
Objectives :
(1) To investigate on the effectiveness in using video demonstration to teach handihaler technique (2) To investigate on any clinical improvement after such a demonstration.
Methodology :
A nurse-led clinic was established for COPD patients in our clinic since Dec 2017. Patients with COPD Group B or above were recruited in this nurse-led clinic, with introduction of Tiotropium Handihaler. Proper handihaler technique were educated by using Ipad video demonstration. The video was produced by the Hong Kong Pharmaceutical Care Foundation and can be freely watch via a Mobile phone Apps. Consent had been obtained from them in using it for education at our GOPC. After watching the video, patient would be advised to do it in front of the nurse to make sure they fully understand and remember the steps. Pamphlet would be given as reminder for patients as well. Alternatively, patient could also download a Mobile phone Apps for reviewing the same video. Patient will be reviewed within 12 weeks for the technique and disease control .
Result & Outcome :
47 patients were recruited in this nurse-led clinic from Dec 2017 till Dec 2018. 31 of them (65%) had already been followed up within 12 weeks so far. 93.5% of them was able to recall and complete all the steps taught in using the device upon follow-up. 17 of them (55%) had shown improvement in CAT score. 12 of them (38.7%) had shown improvement in FEV1. Use of Ipad Video demonstration in educating proper handiler technique is helpful to improve COPD control. We recommend this educational method to be used in all patients using inhaler device.
