Introduction :
According to the data from Sep,2017-Aug,2018 in Tai Po Hospital,56% of patients admitted for hip fracture required assistance in ADL upon d/c home.Carer Empowerment Program(CEP)aimed at equip carers’skills & readiness to take care of patients at home upon discharge.
Objectives :
1)To improve caring skills of carer for patient upon d/c home 2)To improve the carer’s confidence & readiness 3)"Education videos & pamphlet package" could reduce the duration in OT demonstration & facilitate carer to recapture the caring skills at home 4)"Home Environment Screening Pamphlet" could facilitate OT to provided appropriate advice in home modification efficiently & also the carer training.
Methodology :
Pre & Post-survey was conducted.Subjects were 1)main carer of patient underwent Total Hip Arthroplasty/Hemiarthroplasty,2)cognitively & physically capable.Outcome measures was categorized as:1)Carer Evaluation Form,2)Duration in carer training,3)OT & carers'feedback
Result & Outcome :
Result: Pilot phase started from Sep,2018, 10 carers have been recruited.ALL carers showed improvement in 1)caring skills acquired,2)confidence & readiness to take care of the patient upon d/c home,3)with adequate resources in community reintegration for patients,4)adequate use of aids. ALL staff with positive feedback,that can reduce the duration & manpower for demonstration & facilitate OT to screen home environment more efficiently and systematically. Conclusions: CEP was effective in empowering carers' caring skills when patients' upon d/c home,also can facilitate the early discharge of patient safely for saving health care cost and facilitate OT for smoothing the carer training process.
