Introduction :
The hospitalization of children especially admission to a psychiatric ward will create anxiety to both parents and children. By introducing the patient’s journey to patients/carers upon their admission may help them to understand more about their treatment journey and gain their co-operation during their hospitalization. In E2 TMH Child and Adolescent Psychiatric Unit, the patient journey consists of Assessment; Plan; Treatment and Evaluation; Preparation of Leaving Ward; and Discharge. The ward nurse will walk through the whole patient journey with patient and carers upon their admission.
Objectives :
To provide patients and their carers the information of the whole journey to be experienced during hospitalization
Methodology :
Funding was supported by the Patient Empowerment Fund. The funding was used to print coloured pamphlets. The information about the patient journey was all printed in the pamphlets. There were 3 languages available: traditional Chinese; simplified Chinese or English. Different languages were adopted in order to cater patients with different cultural backgrounds.
Result & Outcome :
All admitted cases were given the pamphlet upon admission. All patients and their carers understood the whole journey to be experienced during their staying in the child and adolescent unit. Both patients and their carers gave positive feedback to the pamphlet. Patients agreed the pamphlet could help them to adapt to the ward environment upon admission. Carers expressed that knowing more about their children’s hospitalization experience could facilitate them to alleviate their worries.
