Introduction :
Heart disease is still the top 3 of causes of death in Hong Kong in recent decades. There are increasing service needs in cardiac care units and service expanded in 2018. Community-based education is equally important as in-patient education. "Best For Your Heart" program is an education-based events established by cardiac care unit. Health talks and classes regarding different aspects, such as cooking class and cardiac strengthening exercise class were organised quarterly, several disciplines/experts were invited to be speakers to deliver their specific knowledge informatively and interactively. We collaborated with a NGO for health promotion. We realised that clients can have more motivation in self care management and facilitate in prevention of heart disease.
Objectives :
-To promote self management and build up health concept to community. -To promote understanding on cardiac rehabilitation and related treatments. -To consolidate health knowledge on the care of cardiac diseases and prevention.
Methodology :
Patient discharged from cardiac care unit were recruited. Cardiac nurse would invite the post discharged patients by phone. A series of health talks were organised in a regular basis, for those post discharged cardiac patients and also community, aiming at consolidating their health knowledge. we had cooking class held by dietitians; exercise training class held by physiotherapists; and also we had pharmacist to teach the latest medication knowledge. Totally 6 seminars were organised from 2016 till now.
Result & Outcome :
A total of 161 clients participated in the education program. All participants showed satisfaction to the activities.
Diversified health talks and seminars were effective in health promotion. It helps to encourage clients' engagement and empowerment on self-management and prevention of cardiac diseases. It is suggested to further explore other topics about cardiac care education in order to advance the program.
