Introduction :
LKB, an Extended Care Unit in PMH, provides rehabilitation, convalescent and infirmary care to patients transferred in from PMH. One of our essential nursing care roles is prevention and management of Pressure Injury (PI) regardless it is Community Acquired Pressure Injury (CAPI) or Hospital Acquired Pressure Injury (HAPI). According to PMH Pressure Injuries (PI) statistic from Jan to Oct 2017, it was noted that there was 28 HAPI during 10 months in LKB and among that there were 20 PI was developed in R4S. In addition, enquires and complaints from relatives about patient’s wound condition or progress were received from time to time. Most of the relatives were not aware of patient’s wound condition or progress. Regular proactive communication with relatives to inform and update wound condition by PCLN which enhanced relative’s understanding on patient’s wound condition and progress. Thus this encouraged relative’s participation in care of patient such as brought in patient’s favourite food to boost up nutrition and psychological support which further enhance patient’s morale and wound healing progress.
Objectives :
- To enhance quality of wound management to patients - To enhance staff’s knowledge and skill in wound management - To enhance wound documentation - To enhance communication between nurses and relatives - To enhance the role of PCLN in ward
Methodology :
- Conducted lecture on wound management to update staff knowledge and skill - Conducted audit to monitor wound documentation - Piloted the weekly one day training - one to one ( Nursing Specialist to PCLN) wound management preceptorship program in R4S - Conducted proactive communication with relatives: phone contact to relatives for wound condition update by PCLN in R4S weekly
Result & Outcome :
During Nov 2017 - Aug 2018, one to one wound management preceptorship program was pilot in R4S. One day per week coaching program between nursing specialist and pressure injury link nurse was implemented. Wound dressing technique, use of various dressing material and carer training were introduced and trained to link nurse. Totally 82 patients received the service. 74 patients’ wound conditions and progress were informed and updated to relatives proactively and 20 minutes carer education was conducted at bedside or via phone. All contacted relatives showed understanding and satisfactory for the nursing care and education. No complaint about wound condition received in R4S from Nov 2017- Aug 2018. Total PI is dropped: 46% dropped in LKB and 60% in R4S from Nov 2017- Aug 2018.
