Introduction :
Patient fall was one of the top risks of our department. it was noticed the increasing trend in patient fall incident in 20181Q compared with 2017
Objectives :
1.Enhance fall prevention concept and awareness among A&E staffs, patient and their accompanied person(全民 皆兵 everybody is a soldier) 2.Maintain patient safety in A&E, reduce fall and sustain fall prevention
Methodology :
1.Periodic (once every three months)reinforce staff to assess and identify target patient (e.g. drunk, extreme age, cognitive problem, dizziness, fall history,frequency urination etc)and sharing of incidents to seek for attention of bench-marking on the improvement measures 2.Post up signage in wheelchair and trolley to alert all staff high fall risk case after triage 3.Reinforce and educate patient and accompanied person of fall prevention upon triage 4.Supporting staff engage by perform patient safety round for environmental and conditions check that contributing to fall with checklist recorded 5.Warm remainder message will be broadcast through queuing system and display on LCD screens regularly, as to provide patient safety information for accompanied person 6.Fall Prevention Poster was designed and posted in clinical area for staff and patient as remind and information 7.Portable call bell is available for patient call for help 8.Enhance fall prevention concept for all new staff.
