Introduction :
Princess Margaret Hospital (PMH) started stroke thrombolysis (TPA) treatment for hyperacute ischaemic stroke in 2009 initially as limited in office hour and is a medical consultation model. Now, it is expanded to 24 hours in seven days (24/7) full coverage from 2016 and the service was migrated into coordination by TPA nurse.
Objectives :
To provide people-centred stroke care in professional standard with the well-trained and committed staff in the Acute Stroke Unit (ASU).
Methodology :
1. Multi-disciplinary effort in promoting efficacy of stroke patients.
2. Direct coaching by stroke APN and final assessment by neurologists after 10 potential TPA admission from Accident & Emergency Department (AED) to stroke unit, competency and confidence can booster up.
Result & Outcome :
1. PMH is the first hospital providing 24/7 TPA service in KWC. Successful TPA treatment from 4 cases in 2009 to 96 cases in 2017.
2. More than 14.12% ischaemic stroke patients received TPA up to March 2018, contributing the third place and PMH was the first runner-up hospital in percentage increase in TPA treatment in 2018 in Cluster.
3. KWC was the shortest average length of stay (LoS) of 7.2 days where PMH was 6.5 days in 2017/18. The modified Rankin Scale 0-2 in 3-month time post thrombolysis and Door-to-needle (DTN) both met the benchmark standard. 4. Cluster unplanned readmission rate is second lowest 8.97% among HA hospitals in 2018.
5. With the support from all management level there are total 14 recognised TPA nurses trained up within 2 years.
6. Awarded merit achievement in PMH outstanding team competition in 2018.
