Introduction :
Endoscopes are costly but essential and frequently used equipment during examination/procedure in Ear Nose Throat (ENT) clinic. Endoscope damage is expensive and sometimes non-preventable due to ageing and repeated use. Therefore, it is important to protect our endoscope investment by timely identification of damage with early and promptly intervention. Promptly damage identification always results in shorter repair time and lower repair cost. According to current practice in Specialist Out-patient Department (SOPD) ENT clinic, post damage endoscope handling involve varies parties such as, ENT clinic staff, Endoscopy Unit (EDU) staff and vendors. Communication involves intra and inter-departments. Without detail and clear documentation among various parties could lead to delay and/or mislead way of repair. Our project aim is to facilitate proper documentation and enhance communication among varies parties for post endoscope damage handling in order to promote effective equipment care.
Objectives :
1. To design a comprehensive endoscope damage reporting form 2. To include varies type of endoscopic pictures for better illustration 3. To share the revised form with involved parties
Methodology :
The form design and content was well discussed with varies parties including ENT clinic staff, EDU staff and vendors. Precise endoscopic pictures of different kinds of endoscopes were provided by their own vendors to be included in the form to ensure accuracy. The new form was shared with involved parties with detail explanation. Intra-department sharing was also done by involved parties to ensure correct documentation. The new form was piloted among all concerned parties in December 2018. After implementation of new form, evaluation forms were distributed to involved parties for outcome measurement.
Result & Outcome :
The details of damaged endoscope were clearly stated in the newly designed form with using the precise pictures for illustration. The form also acts as a checklist to guide staff to follow the flow of post endoscope handling and keep tracking of the repair progress. It also decreases handover time, facilitates easy progress tracking and promotes effectiveness of endoscope repair. The evaluation forms were completed by involved parties. Likert scale was used to evaluate the result. 100% excellent score marked from the users in all four aspects: user friendly of reporting form, better communication, clear documentation and timely the repair process. Endoscopes are valuable equipment in ENT clinic. When damage is non-preventable, timely accurate repair is essential. A comprehensive reporting form enhances the repair process and promotes effectiveness and efficency.
