Introduction :
Kowloon West Cluster consists of 13 community General Out-Patient Clinics distributed in various districts, including Shamshuipo, Kwai Chung, Tsuen Wan and Islands. With the purpose of proactively identifying and improving risk areas in different clinics, our department organized a program of safety round within the 13 clinics. The key focused assessment areas included medication safety, patient privacy and workplace violence.
Objectives :
To share good practice with each other in different clinics To identify risk areas and improve them by providing different sources and measures. To promote risk aware culture and enhance staff knowledge of risk management.
Methodology :
A working group was established with different rank of nurses from different clinics and a clerical staff. The compliance checklists for medication safety, patient privacy and workplace violence were developed. All members were invited to visit the 13 clinics and participate in assessment, as well as input their comments.
Result & Outcome :
According to the collected feedbacks and with reference to individual good practice, the different improvement measures were created and consistently adopted in all clinics. Improved areas of medication safety: - Temperature control of medication refrigerator - Medication partition and labelling - DDA storage - Store control - Medication documentation - Medication information references. Improved areas of patient privacy: - Placement of medical records - Transportation of medical records - Disposal of confidential documents - Information security. Improved areas of Workplace Violent - Emergency escape floor plan - Fixing and fitting of dangerous object. Staff satisfaction survey: - 100% of members agreed that the program is useful in work - 100% of members agreed that the program is well organized. - 100% of members agreed that the program should be organized more in the future.
