Introduction :
Aging population may rise various health caring issues especially the frailty skin. The broken skin may cause painful, infection and immobility, as a result to cause patient decondition, Therefore, skin and wound nursing round have been developed in geriatric setting to enhance the awareness the skin care among the nurse
Objectives :
To protect the frailty skin and prevent wound injury among elderly patients To provide holistic nursing care for frailty elderly To enrich the nursing staff on skin caring skill
Methodology :
1) Established Wound Care Committee to assist the nurse or health care workers to care the frailty elderly 2) Setting up a nursing round weekly to coach and supervise the nurses to mange complicated skin management and enhance better communication between the staff 3) Ward nurses who are graduated over 5 years will allocated to wound team for 4 weeks to enhance their knowledge of wound care 4) Develop education session to refresh staff's wound care skill 5) Evaluate the process of wound round
Result & Outcome :
Development and implementation of skin and wound round can improve quality of care to elderly people. With the collaboration with wound team, our staff can enhance their knowledge and confidence to manage the difference types of skin injury. Staff agreed that skin and wound round could empower the competence of managing wound care. Nurses also found job satisfaction in this program regarding to better skin enhancement. As a geriatric nurse, we play an very important role in protecting and caring our elderly patients. We believe that better communication and experience sharing via nursing round can enhance the awareness of skin or wound care.
