Introduction :
Hospital-acquired pressure injury (PI) on heel region are commonly occurs in long care facilities especially for patients with poor mobility, lower limb's contracture and poor cognition. Heel, becomes one of the most perpetually affected anatomical regions for developing pressure injury for the high dependency patients. Heel ulcer also associate with increased risk for pain, reduce mobility, systematic infections and even death on patients. Therefore, multi-disciplinary team approach for treating and preventing pressure ulcer on heel is essential. Moreover, there is currently a great diversity of heel protector boots design among cluster's hospitals while there is limited evidence to support the efficacy of the conventional designs. Hence, pilot of intra-cluster quality improvement program involving early risk-stratified interventions, new product design, empower of skin-assessment practice & staff education and modified the product's sourcing method have been launched by Occupational Therapy and Nursing Department in Cheshire Home & RTSKH.
Objectives :
1) To standardized the risk-stratified intervention guide through evidence-based analysis of various heel protector’s boots by conducting pressure mapping by Occupational Therapist (OT). 2) To evaluate the efficacy of the new heel protector boots design by measuring their pressure distribution performance 3) To evaluate the feedback among nursing staff (PI link-nurse) on the effectiveness of the new protective boots design and the stratification guide. 4) To evaluate patients/caregivers’ satisfaction for the new design of heel protector boots 5) To align OT practice on prescribing heel protector’s boots for high dependency patients 6) To evaluate the organization benefits to CCH for the new sourcing logistic of heel protector boots that supported by Occupational Therapy Department in RTSKH.
