Introduction :
Nurses use the clinical cam App in iPad for taking clinical photo in 4 wound clinics (West Kowloon GOPC, Lady Trench GOPC, Tsing Yi Town GOPC & Ha Kwai Chung GOPC), KWC FM & PHC since Oct 2018. Photographing wounds provides a visual image in order to accompany wound assessment in the nursing notes, Clinical Management System (CMS). Wound photos taken at intervals during the care process can provide evidence that the wound was regularly assessed and staged. They can show either a progression of healing, or at least show how treatments were introduced to address a non-healing wound.
Objectives :
To provide a visual, objective interpretation for wound assessment and document it’s progress of healing. Also improve communication among nurses and doctors in patient’s wound care.
Methodology :
Using Clinical Cam App in iPad taking clinical photo in wound clinic since Oct 2018 and photos upload to patient album,CMS. Clinical wound photos will be taken at intervals during wound care management in wound clinic. Nurses in wound clinic and Doctors can track the wound progression in the patient album, CMS
Result & Outcome :
Wound photography is vital to the accuracy and objectivity of wound documentation in wound clinic, KWC FM& PHC. It is objective interpretation of clinical photo such as slough, eschar, granulation tissue or undermining. The photo clearly identifies areas within and around the wound as a comprehensive view in wound documentation. The primary clinical goal is assess whether a patient’s wound progress. Also highlight the non-healing wounds that review the case for case discussion and wound care management plan. Hence, improve care outcomes for patients by providing an extra communication tool between in collaborating care among nurses and doctors. Staff’ satisfaction survey for wound photography was conducted. 99% of nurse and doctors agreed that wound photography taken by iPad via Clinical Cam App facilitates an evidence-based practice of wound care management and improve the communication between nurses and doctors for patient's wound care.
