Introduction :

Department Specific Hand Hygiene Enhancement Program (Program) has been implemented in selected departments since 2016 and the overall hospital Hand hygiene (HH) compliance rate could be sustained at above 75%. In order to keep the momentum on sustaining the good HH practices, a Cross Sectional Study was executed to evaluate the effectiveness of the Program.

Methodology :

The Program was implemented in 2 selected clinical departments (Group 1) and 2 non-clinical departments (Group 2) since 2016. The program includes: i. Meet with department representatives to identify department specific HH moments ii. Provide lectures on general HH knowledge, department specific HH moments and address rationale on the importance iii. Conduct HH hands-on exercise, return demonstration with immediate feedbacks via florescent lotions game. iv. For Group 1, an additional stimulation training on HH Observational Survey technique provide to select staff with validation on HH observational skills was provided.

Result & Outcome :

Results and Outcome 1. Improvement on HH compliance noted among both groups. Statistically significant improvement on HH compliance was noted in Group 1. Group 1 increased from 62.7% to 80.2% (p=0.006) while Group 2 from 50.8% to 64.3% (p=0.199). 2. Program with addition training on HH Observational Survey technique had a significant effect on improving HH compliance (OR: 2.706; 95% CI: 1.1097, 6.675; p=0.050). 3. Decrease in MRSA hospital acquired infection (HAI) rate was noted from 0.793 per 1,000 patient bed days before the program to 0.404 at after. 4. Positive staff feedbacks received. Conclusions Department Specific Hand Hygiene Enhancement Program can improve HH compliance among healthcare workers and MRSA HAI rate in hospital. Skill transfer on HH Observational Survey technique can empower frontlines to provide onsite supervisions and sustain awareness among peers in workplace. It is worthwhile to invite departments on implementing the program at different time point to keep the momentum on sustaining the good HH practices in hospital. Hand hygiene is the single most important, simplest, and least expensive means on reducing the HAI and the spread of MDRO. Generic training Hand hygiene promotion program apply throughout hospital may not be the best method on sustainability.
