Introduction :
Administration of Intravenous fluid (IVF) is very common in Department of Surgery. Other than 5 rights checking during administration, placement and labelling of IVF in storage area are also important to facilitate nurses to take the correct IVF, so that medication incidents can be prevented. There were several medication incidents related to misplacement of IVF in the past year. It has been finding that the labelling in the storage area is not clear and big enough, and the labelling in each surgical ward was different. In order to prevent medication incidents, 5S management of the IVF storage area has been used in the Department of Surgery since 2 May 2018 by organizing the work space in a clean, efficient and safe manner to enhance the productivity, visual management and standardization. According to “set in order” and “standardize” of the 5S management, setting up standards for a neat workplace can be done by eye-catching color coding and clear labelling. By doing that, it can facilitate the staff easily identify the correct place of the needed items, which in turns prevent medication incidents.
Objectives :
To prevent medication incident related to misplacement of IVF in Department of Surgery.
Methodology :
1. Identify different types of IVF used in ward. 2. Decide the color coding for each type of IVF. 3. Design and re-arrange the location of the IVF. 4. Design and print the IVF labels. 5. Carry out pilot trial of the color code labelling in one of the surgical ward (ward 8A) in 2 May, 2018. 6. Review the effectiveness of the color code labelling by asking staff comment after 2 weeks of pilot trial. Rearrange if necessary. 7. Promote to other three surgical wards to use the IVF color code labelling in June, 2018.
Result & Outcome :
There is no medication incident related to misplacement of IVF was reported in four surgical wards since the commencement of the project. Also, all staff in the four surgical wards are satisfied and happy with this new measures.
